Community resource for members of our vulnerable population

The Tempe Police Department’s TEAM Reunite (Tempe Endangered And Missing) Program is a community resource service provided for members of the vulnerable population community who work, live, study, or receive treatment within the City of Tempe. This would include, but not be limited to, children or adults who have:

  • Autism
  • Down Syndrome
  • Dementia / Alzheimer’s
  • Traumatic brain injury

This program will provide parents and caretakers with a mechanism to share crucial information with police officers who are responding to calls for service involving these individuals.  Information on the vulnerable person, their parent(s) or responsible caretaker, and another safe contact person will be collected through the form below. Current photographs of the registered participant are requested to assist in locating the registered person, if he or she is ever missing.  To register, please complete the following steps:

  • Download and complete the TEAM Reunite Registration Form.
  • Provide two clear, well-lit photos of the person to be registered (one head-shot photo & one full-body photo).
  • Seal your documents and return them one of two ways:

Further questions can be directed to the T.E.A.M Reunite program at teamreunite@tempe.gov  Police officers will only be able to access this information when able to do so safely. Considerations of weapons, officer safety, and speed at which factors are changing must be considered on a case by case basis.  Furthermore, all participant information will be available to certified law enforcement officers only.
