Step One: Have the following documents ready or in process before applying

A Transaction Privilege Tax (TPT or sales tax) License is required and must be renewed annually for the business
The Amusement Transaction Privilege Tax is due on the gross revenues for operating or conducting theaters, movies, operas, shows of any type or nature, exhibitions, concerts, carnivals, circuses, amusement parks, menageries, fairs, races, contests, games, billiard or pool parlors, bowling alleys, skating rinks, tennis courts, golf courses, video games, pinball machines, public dances, dance halls, sports events, jukeboxes, batting and driving ranges, animal rides, or any other business charging admission for exhibition, amusement, or entertainment.

Step Two: A Tempe Amusement Machine Operator License is required and must be renewed annually for the business and must include an application for the following to be complete:

the business where the machine is located
If you need a Tempe Regulatory License, you will need to renew before January 1 each year.

Fees
$20 annually per machine, pro-rated quarterly if a machine is newly licensed after each calendar quarter

We accept check by mail or in-person. We do not accept cash.
