Risk Management is responsible for city employee safety and industrial hygiene, as well as processing city employee worker's compensation claims.  Risk Management also processes property/liability claims against the City of Tempe.

To file a claim against the City of Tempe, complete the Notice of Claim Form.  Please note that Arizona state statute requires that claims must be filed with the City Clerk's Office within 180 days after the cause of action accrues.  It is advisable to hand deliver a claim by appointment, to avoid extended wait times, to:

To request an appointment, please call the City Clerk's Office at 480-350-8242, or email clerk@tempe.gov.  Please leave a detailed message to include your name, contact number, and reason you need to visit the office (Notice of Claim, lawsuit, etc.)
