Bus Stop Maintenance Request Form

Please fill out the form below to aid the City of Tempe in identifying bus stops and shelters that are in need of repair. While we do our best to service every bus stop in a timely manner, problems do occasionally occur in between scheduled maintenance. Thank you in advance for your participation in this program.

2) What is the closest cross street (e.g., south of Southern Avenue) to the bus stop you are reporting?
3) What is the issue/maintenance needed at the stop?
