Welcome to City of Tempe Special Olympics!
Youth (8 yrs+) and adults with intellectual disabilities are invited to join Special Olympics Arizona to learn sportsmanship and teamwork while achieving their personal goals! For more information about the City of Tempe’s Special Olympics programs, please contact Josh Bell, Recreation Coordinator, at 480-858-2469 or visit our website at www.tempe.gov/adaptedrecreation. A Special Olympics Medical Release Form signed by a medical examiner (good for three years) is required for all athletes to participate, as well as a City of Tempe registration form. There is a $10 registration fee for each sport. Some sports, such as aquatics, will have an additional uniform fee due to the cost of the uniform. Please refer to the registration flyer. Registering in advance is recommended. You can register in advance one of the following ways:
- Register online at www.tempe.gov/brochure (and use the registration code for the program).
- Mail registration to Attn: Josh Bell, Adapted Recreation, 3340 S. Rural Rd., Tempe, AZ 85282.
- Register in person at the Recreation Office, 3500 S. Rural Rd. (2nd floor of Library), Pyle Adult Recreation Center, 655 E. Southern Ave., or another City facility that processes registrations.
Medical Release and Consent Form
All Special Olympics athletes are required to have an up-to-date Medical Release and Consent Form on file with the City of Tempe delegation and Special Olympics Arizona. This document is good for three years from the date of the doctor's and participant's signatures. Athletes without an up-to-date Medical Release and Consent Form will not be allowed to participate in Special Olympics competitions. The form can be downloaded below. You may submit the form in person or mail to City of Tempe Adapted Recreation, 3340 S. Rural Road, Tempe, AZ 85282.
